Pharmacogenomics is the study of how your genes affect the way medicines work in your body. In plain terms: because everyone inherits slightly different versions of the genes that process drugs, the same pill at the same dose can help one person, do little for another, and cause side effects in a third. A pharmacogenomic (PGx) test reads those genes.
Pharmacogenomics, defined
Break the word apart: pharmaco (drugs) + genomics (your genes). Put together, pharmacogenomics answers a single practical question, how is your body likely to handle this medicine, given your DNA?
Most drugs are broken down by a handful of liver enzymes. The genes that build those enzymes come in different versions. Depending on which you inherited, you might clear a drug quickly, slowly, or normally, and that changes how well it works and how safe it is.
Why the same drug affects people differently
Imagine three people take the same standard dose of the same medicine:
- One clears it too fast, so it never reaches a useful level and seems not to work.
- One clears it too slowly, so it builds up and causes side effects.
- One clears it normally and does fine.
None of them did anything wrong. The difference is largely genetic, and that is exactly what a PGx test measures.
A few real, well-established examples
Pharmacogenomics is strongest for a specific set of gene-drug pairs backed by CPIC guidelines:
- CYP2C19 and clopidogrel: a common blood thinner after heart stents. Some people don't activate it well, weakening its protection.
- SLCO1B1 and statins: certain variants raise the risk of muscle side effects from cholesterol drugs.
- TPMT and NUDT15 with thiopurines: used in IBD, autoimmune disease and leukaemia; some variants make standard doses dangerous. NUDT15 variants are notably more common in Indian and Asian populations.
- CYP2D6 with codeine and some antidepressants: changes pain relief and drug levels.
What a PGx test can and cannot do
It can:
- Predict how you are likely to metabolise or respond to certain drugs.
- Help your doctor pick a safer starting medicine or dose.
- Explain past bad reactions or medicines that "didn't work".
It cannot:
- Cover every drug, only genes with solid evidence.
- Diagnose a disease.
- Replace your doctor. A PGx report is information that guides prescribing, not a prescription. Never change or stop a medicine on your own, your doctor decides.
